Understanding Cervical Insufficiency & Pregnancy After Loss

I lost my first at 19 weeks, we thought it was because of a incompetent cervix, but when the pathology report came back. I lossed my baby because of a infection(Chorioamnionitis).I started spotting on Wed, which I then went to the hospital, they checked the cervix, and is was closed, but then again I spotted on sunday and I had massive pains, like labor, that is when I lost my baby. I am pregnant again, 14 weeks. my doctor checks every month for infection, and also my cervix every two weeks to make sure its closed, everything is good so far. I am still scared, I just don't know if I have a incompetent cervix. Is it safe to have the cervical cerclage if my cervix starts to open around 19 weeks? Also how fast the the incompetent cervix start to dilate, does it take weeks or just happens instandly?

The cause for the first preterm delivery at 19 weeks was due to an infection and was not due to an incompetent cervix. We usually do not insert a cerclage unless you have lost three consecutive pregnancies. However, it is a good idea to check your cervix every two weeks. As long as it is closed, you do not have an incompetent cervical os. If you do go into preterm labor, and start to dilate, it can rapidly progress to full dilatation and you can go into labor. It is doubtful that this will occur. However, if you start to notice ANY leakage of fluid or start to have any regular contractions, contact your doctor to be checked. If the decision is made to insert a cerclage, it can, of course, be done safely at any time, as long as there are no fetal membranes dropping down into the vagina and you are not dilated more than 4cm.
