Uterine Hematoma After Trauma: Understanding Risks & Management

hi, i just found this website while doing some research for my daughter, She is 23 weeks pregnant .this is her 3rd pregnancy. her first baby, a daughter was a vasa previa baby born by emergency c section and her second baby boy was born natural(though she had placenta previa throughout most of the pregnancy.
this was a normal pregnancy(the placenta was up high in the back)
5 days ago, she had a fall at work, and went to emergency with pain and cramping, she was told at first it was a pulled lover abdominal muscle, she went for an ultrasound today and found out that she has a hematoma in her lower front abdomen, i guess on the uterine wall. she goes back in 2 days for another ultra sound, i guess to see if the bleed has gotten bigger and to check her blood?
the doctor seems to be concerned, this seems very scary that she might go into premature labour or something. the baby is too small,
what do you think about all this, could she have ruptured at her c section incision site, tearing it as she fell?
how dangerous is this? and what are the chances for babys survival at this time. is there anything that they could do?

If the old Cesarean section scar have ruptured, this would be clear to the caring Obstetrician during examination and pregnancy will not be allowed to continue. This rupture may occur without any bleeding or pain and that's why it is called silent rupture.

On the other hand, a hematoma of small to moderate size can be followed up with serial ultrasound scans and pregnancy may be allowed to continue if the scans showed diminution of its size provided that the baby is in good condition. Any deterioration in the fetal status will require an emergency Cesarean.

My advice is to prepare your daughter to deliver in a well equipped hospital with adequate neonatal intensive care facilities should a preterm delivery deemed necessary. It goes without saying that senior obstetrician should be available to carry out the surgery.
