HCG Trigger & OPKs: Impact on IUI Success - Fertility Forum

Question
QUESTION: Hi im TTC since 8 months .Im 35 DH is 40. all our tests( blood work , HSG SA with strict morphology r normal). I tried 2 cycles of femara as clomid thinned my lining.
I took femara 2.5 mg BID from day 3-7. on day 10 my U/s showed 2 follicles on right measuring 20mm and 22mm and one on left measuring 16mm. I took the hcg trigger shot on day 11(31st jan 09) at 9.00pm and have my IUI on day 13th(2nd feb 2009). my question is does the HCG trigger interfere with opks as my opks are still negative today on day 12. Also how many hours after the trigger does ovulation occur...thanks

ANSWER: Hello,

Yes, HCG injections will show the OPK to be positive as it is testing for the LH surge and HCG cross reacts.  My concern in your case is that everything was delayed.  The HCG should have been given on cycle day # 10 and the IUI done on day # 11 and/or 12.  I would expect ovulation to occur around that time.  Cycle day # 13 might be too late.

QUESTION: I had my very 1st IUI today(feb 2nd) at 11:40am.

I was on femara from CD3-CD7 (23rd jan-27th Jan)
went for follicle u/s on CD10(30thjan)- had 2 follies measuring 20mm and 22mm on right and one measuring 16mm on left. uterine lining was 13.6mm.

Took trigger shot on CD 11(31s jan) 9.00pm
my opks have been - since CD8 however on the morning after trigger CD 12 they looked a little darker although still lighter than control.

I woke up this morning CD 13 and took my temps and there was a big jump from 97.22to 97.97..i thought i ovulated and went to my gyn to make sure i did with another scan . she scanned me and said u have ovulated but this is the perfect time for IUI (38hrs after trigger) which i thought was strange coz i had already ovulated and how would that do any good??
anywayz she did the IUI and while doing this she said "ur cervical fluid is very thin and stringy and this is what we want to see during IUI.
Do u think this cycle was a bust becoz of late IUI or du ui still have a chance of hope this cycle

TTC since march 2008
All blood work normal
DHs SA with strict morphology normal
HSG in dec 2008 normal
tried 2 cycles of femara before IUI

Answer
Hi Again,

All we can do at this point is hope for the best.  Since we don't know exactly when ovulation occurred, it could have occurred minutes or hours before your visit with the doctor and the IUI, there is still a chance.  In general, we want the IUI to preceed ovulation.
