QuestionHi my name is monica I have an 18 year old daughter who recieved an adnormal pap which lead to coposcope and punch biopsy were almost positive she has hpv cin 2 which is high risk for pre cancer cells .if im following her doctor now they want to due a cone biopsy any information on this would be greatly appreciated .we are seeking 2 more consults from different gyn's because she wants children and we were worried about the leep procedure and the skip right pass that and im not sure why any information would be helpful

AnswerDear Monica, It is always wise to get a second opinion and even a third if you feel that third one is necessary... All OB/GYN's are different and each prefers to do what test they feel will get right to the heart of the problem.... HPV with CIN 2 (moderate dysplasia) is more likely to spread/grow than possibly regress like with CIN 1..... CIN 2 usually means that at least 1/2 of the cervical epithelial layer is ABNORMAL... Why this doctor wants to skip the leep procedure and go right to the cone procedure is basically his/her call and you would really have to ask the doctor as to why this is his/her decision.... Unfortunately, too may people will not ask the doctors the questions that need answers, and if you do not, you can search and search for another opinion and still not know what to do or what is considered correct procedures....
The doctors/lab "MUST" determine the GENETIC TYPE of the HPV because some genetic types are more dangerous like HPV 16 and HPV 18.... This may also be why some doctors prefer one test over another because they need to get a GOOD section of tissue, get as much tissue as necessary for "margin" testing, etc..... I just really can't say why doctors choose their testing procedures the way they do, but I would suggest that you talk truthfully and totally with her current doctor before running to 3-4 more doctors, but definately get a SECOND opinion, but that should be all you need provided that you talk openly and ask questions of her current OB/GYN and the second opinion doctor....
