Questionsir age 45 weight 100kg male india 168cm
sir since 2 years for mild diabetes dr has presceibed 1000 mg SR evening for 1200 to 1500 cal diet in 3 or 4 divided times.Bit it is not possible to follow strictly regularly 1200 to 1500 cal. some times due to winter only on one seating 1500 cal is eaten.under such circumstances can we use occassionaly xenical type or acarbose type drug?one doctor has permitted while other has told that xenical occassionaly can be taken But for acarbose use occassionay they have not experimental data.what should do under such situation?while on fasting can we stop medicines for that day?

In the USA we typically prescribe an 1800 kcal diet. Also would suggest daily exercise of both aerobic and resistance types. Would normally have my patients on Glucophage (Metformin) 1000 mg twice daily and would occasionally use acarbose type medications but not usual. I NEVER use Xenical as I think it is a bad drug not just for diabetics but for all persons. Just lower saturated fats and increase Omega-3 intake. Make sure you use a dietary supplement such as Glucose Support (see www.vitasanus.com for description). Would not necessary recommend stopping Glucophage while fasting since it does not usually cause hypoglycemia.
