Understanding a 4cm Ovarian Cyst: Symptoms, Diagnosis & Next Steps

Question
I've been having lower right abdominal pain for the last month and a half. About a week ago, my doctor sent me to get a CT scan and they found a 4cm cyst on my right ovary. I also had an ultrasound to make sure that there was no torsion. The doctor said the cyst was really no big deal and asked for me to come back in 2 weeks for a follow-up ultrasound and that the cyst would probably go away on its own in the meantime.

The past week has been awful. I have a constant dull ache in my lower pelvic area and severe stabbing pains at random times in my abdomen and lower back. Sort of like a hot knife being jabbed into me and dragged across. The only thing that seems to calm it down is to lay down with a hot water bottle on the area. From what I can understand, a 4cm cyst is nothing to worry about. Tiny thing. Not big enough to be removed. But why is it hurting so much? From everything I've read and been told about cysts, there really shouldn't be any symptoms unless they're huge. My doctor really wasn't concerned with it either at the time so I kind of feel stupid complaining about it to her. Should I wait another full week to go in?

Answer
there is a lot of room in the pelvis... babies fit in there.  A little 4 cm cyst fits easily... it is a question of whether it is stretching the cyst wall.  A lot of them hurt because the fluid irritates the pelvis when they rupture, which yours hasn't.  You might need to look for another cause of pelvic pain, such as endometriosis or infection.  It is sometimes really difficult to diagnose pelvic pain.

If there is no fever, vomiting, getting worse, then you can wait a little, as the cysts tend to come and go, but if things are worsening.... get it evaluated again.
