Tissue Expander Issues: Understanding Upside Down Placement and Surgical Options

I had a tissue expander placed after a right breast mastectomy.  After being filled with saline, my plastic surgeon said it was upside down and he would have to cut muscle in several places in order for an implant to fit.  Therefore, isn't it correct that the tissue expander served no purpose?
