6-Month Mammogram Follow-Up: Dense Tissue & Questionable Density - BI-RADS 0

QUESTION:   On Monday morning I had a follow up 6 month bilateral digital diagnostic mammo w/cad for a probable benign finding in left breast. The findings were as follows: , both breast tissues are extremely dense. There is a questionable asymmetric density in the inner right breast at posterior depth which questionably persist on additional views also , there a numerous scattered micro calcifications present bilaterally. BiRads 0 - they then insisted on doing an ultra sound immediately because of the asymmetry right breast. Findings are as follows: , real time ultra sound was performed on areas of interest in both breasts. There is an irregular hypoechoic solid nodule in the right breast at 11:00 3 cm, from nipple which is superficial measuring 8mm. There is a stable hypoechioc nodule in the left breast at 1:00m 1 cm from the nipple measuring 6mm.  There is a stable, probable complex cyst in the left breast at 5:00m 3 cm from the nipple measuring 4mm.  IMPRESSION: Suspiciuos of Malignancy, birads 4 Bilateral MRI of both breast prior to final decisions on biopsies. The ultrasound was done right away on Monday morning, unfortunetly the MRI had to go thru the insurance co. so that was done on friday. By the time I left my appt. on Monday. The office had me set up for the mri, I had to see a surgeon that they paged right from their office. Is it pretty safe to guess that this outcome will not be all sugar and balloons, I have already had two biopsies done on the right breast, both benign but show rare ductal and para cortical unusal hyperplasia. I also had a biopsy done on Wednesday but it was a ultrasound guided needle biopsy and it was only done on the right outer side  at about the nine o'clock space. I pretty much have figured it won''t be so great, but what are you gonna do
 


ANSWER: In my opinion ALL these lesions seen by ultrasound should have been subjected to ultrasound guided needle biopsies immediately! Why wait with such a rather simple procedure?! What it is that has been seen will not be KNOWN until biopsy results are in! All radiological evidence (including MRI - though it is better than most other things)is just indicative NOT a final diagnosis! It is probable that there MAY be something there but it is not sure! So I suggest that you push for biopsies. Please do keep me posted.

QUESTION: Ok,so on wednesday I had only ONE needle biopsy on the upper outer right breast where four samples of tissue were removed. of course a holiday weekend and I have not received anything from pathology. The biopsy was done with an ultrasound as a guide. If I am not mistaken should'nt there have been biopsy on each area mentioned in the report.  If in fact that is the case should I push for the biopsies to be done, or should I just pick up all my films and go to the oncology specialist located at Lutheran General Hospital.  I don't want to appear pushy, but this is my health, and I would really appreciate a non bias guidance

Yes you are right! All suspect lesions should be subjected to biopsies. I think you should push for them but if that does not work go to the oncologist! Good luck!
