QUESTION: Dear dr Pat G.
Thanks for your concern. Yes in Egypt we have the decision to take the medication or not. Howover, I took the Methortexate injection(50 mg) three days ago.I'm fine, the light bleeding and the discharges disappeared yesterday. I'm fine as I told u ,the only problem is the hormone slow withdrawal. I feel ok, no cramps, no bleeding. The doctor told me no problem of just one injection.
Thanks a lot for ur help and suggestion about adoption,but it is not possible in Egypt. Could I ask where you are? Now I have frozen embro in the hospital. Do u think it is a good decision to try transfering them three months later.

Please know that I am not a doctor. I am just a woman who has done a lot of study and talked to a lot of women. When I need information, I have doctors and other people I can ask, and I can also get information off the internet. I live in the United States. It doesn't matter to me where you live, or what the color of your skin is, or any of that. You are a human being, and I esteem all human beings equally. And in fact, I have personally welcomed people of other colors, cultures, and so forth, into my family. My grandchildren include children who are fair skinned with blond hair, and children with medium skin and black hair. We have four adopted children in our family. We also have five other children and seven other grandchildren. I dearly love all of them.

Doctors will tend to tell you that whatever they decide to do won't be harmful. Otherwise, they wouldn't do it. That said, you asked me about the embryos. Since they already exist, and you are willing to take the risks involved, by all means do go ahead and try to transfer them. They are your children. It would be wise not to have more than two or three transferred at any given time, and it would be wise not to select some to survive if all of them are able to implant and grow. Any procedure to reduce the number also carries risks, and could harm the ones left behind. It may mean that you will carry twins or triplets, and that the burden on your body will be greater. But if you take good care of yourself, and eat a very nutritious diet, it is certainly possible. Here, there are times when some frozen embryos have not been implanted into their original mother, and other women have adopted them, and had them transferred. They are called Snowflake Children, and some of them have testified before Congress about their rights.

If you do think about adoption later, it may be possible to adopt a child from another country. The only question would be whether or not you will be able to do this legally in your country: bring that child in and become the legal parents. For that purpose, it will be good to consult someone who knows about such things. Also, there is the possibility that you could move to another country where this will be allowed.

I'm again pregnant with a twins after six months of my first abortion. I'm in week ten.My problem is the internal OS is 8 mm, and the length of the cervical canal is 40 mm.Could u tell me am I at risk for abortion again.

Congratulations!

A pill abortion doesn't make the cervix incompetent, unless there is actual damage. If a woman's uterus can rupture (which does happen), the cervix would probably be able to be damaged as well, but I'm not reading about this in the literature anyplace.

I did a little research. Doctors aren't willing to go on the measurements of the os and cervical canal prior to 12 weeks. For one thing, the length of the canal varies during the first few weeks. They have found that when the length of the cervical canal is less than 25 mm, there is a risk of very premature birth. This applies to pregnancies with twins as well. At the moment, because your canal length is 40 mm, it doesn't sound like you are at risk of losing the babies. Don't let them talk you into taking action to get rid of them. It sounds like your doctor may be taking good care of you, since he is watching and reporting on such things. He will probably know about using cerclage to make sure the cervix can continue to support the weight of the babies, and it sounds like he will be aware if there is a problem.

When your babies are born, please send me a picture. :)

Thanks for ur reply. I have more than one doctors. One of them insists in cerclage simply because I'm pregnant with twins;the other said that cerclage caused problems and might lead to abortion.They both are worried about the internal OS which is 8 mm. What do u think?Can I go without cerclage.Does that process have side effect.Anyway I have two wait two weeks and make an ultrasound again.

The most I can do is suggest a lot of things to think and ask about.

I'm one of those people who says don't mess with it if you don't have to. Women carry twins all the time.  Ask the doctor who wants to do cerclage to show you some studies that show that carrying twins with an internal os of 8 mm is likely to lead to miscarriage. Cerclage is normally used when miscarriage is threatened. It is done because they want to prevent miscarriage. The other thing about cerclage is that you have to have it removed in time for birth. While we may be able to count on having doctors there who can remove it, I probably wouldn't be quite that trusting. I wouldn't want to put myself in a position where I couldn't give birth without intervention.

I think part of the problem may be that when you have the extensive intervention you are having in order to have children, the doctor you work with is going to be the kind who wants to intervene. He also doesn't want failure, and I don't blame him. You might also ask him if they have learned that cerclage is more necessary in situations of in vitro fertilization. Also, the doctor may be nervous because you lost twins before.

Ultimately, you will have to decide which doctor you trust more. But generally speaking, the ones who do less to a person also cause less harm. If you want a third opinion, you can always get one.

Since I can't examine you or anything, I can't express an opinion beyond that.
