The Importance of Client Consultation Forms for Sports Therapists

Could you kindly please explain to me why Client Consultation Forms are so necessary / important for a Sports Therapists?

Even though your question is slightly out of my expertise as I am not a Sports Therapist, I will try to answer your question since there apparently are no "Sports Medicine" volunteer experts listed in allexperts.com.

Any health professional whether it be a doctor, physical therapist, massage therapist, personal trainer, etc. usually would be "lost" without some kind of "Client Consultation Form" on the initial visit.  One such type of form can be seen here:

This type of form will usually provide enough information for the professional to determin whether to proceed with treatment/care for the client or not proceed and refer them out to someone else.
