Ontario Bariatric Referral Process: Information for Physicians

Question
I was instructed to take the referral form to my physician to be completed then submit it to the registry. I can not find the referral form for Ontario, can you help me?

Answer
The form it seems has been taken down now that the registry has been put up. Your doctor will need to go through the bariatric registry site. He must register and perhaps then either fill out the form online or he may have access on the site now.
