Understanding Bleeding in Early Pregnancy: A First Trimester Guide

Question
I am 29.  I have no children and never been pregnant...before now.  My husband and I decided to go off the pill in July (so last BCP was on Sat, July 1).  I had a period on Thurs, July 6th (just like for all the other years...trusty 'ol Thursday) I had my next period on August 4.  Next on Aug 30.  Then Sept. 24th which was my last period.  Following that perios, we officially "tried" following the octors advice of trying to have 3 periods off the pill before conceiving only to familiarize myself with my cycle.  Well, since I had NEVER taken a pregnancy test in my life and I am a preparer by nature, I bought a double test.  We took a test on Oct 17th (more for the fun of it and to see what it was like)and it was negative.  The box said it could predict for up to 5 days before missed period.   Anyway, on Friday, I still had no period and that evening we were going to dinner with friends and we always split two bottles of wine.  As a safety percaution, I took a test before dinner and to my surprise, it was positive (though the second line was faint) I went out and bought an EPT and it was a plus. Soooo excited.  Well, I figure with this info. that on 10/27 I was one day shy of five weeks.  I started bleeding.  First red, pink, and clear mucus(just once), then brown three times on thurs and 4 times friday.  More than spotting but not enough to soak a pad.  The heaviest was when I went to the bathroom and wiped.  My question is about first trimester bleeding.  This is something I've never heard of.  The nurse on the phone at my OB office said it could be implantation bleeding but wouldn't that have occured BEFORE the 21st?  I am not asking for a diagnosis...just info. from your experience on pregnant women at week 5.  I appreciate any info. you can provide sine I do not see the doctor until November 14th. (that feels like forever when you are bleeding...and I just feel like any bleeding can't be good)thank you

Answer
If your last period started on September 24, you would not have ovulated (and conceived) until October 7 and could have conceived if you had sex on October 6,7,8, or 9. If you did become pregnant at that time, a pregnancy test would not show a positive result until 14-21 days later (no matter what any pregnancy test manufacturer's claimed). The best time to take a pregnancy test is seven days after missing your next period. The test could have been positive any time after October 20. On October 27 you were 4 weeks 2 days gestational age. Prior to 6 weeks (from the first day of your last period) a sonogram will only show an empty gestational sac but no fetus or fetal heartbeat. After 6 weeks, a sonogram shows a fetus and a fetal heartbeat. A small amount of vaginal bleeding is common in early pregnancy. Often, you see brownish blood which is "old:" blood that is being exuded from the uterus by the growing fetus. If the bleeding is heavy with cramps, it could be the start of a miscarriage (which occurs in 15-20% of every pregnancy). If it is only seen when wipin and is not copious, it probably is fine. A sonogram at this time will not help. Wait until you reach 6 weeks from your last period, and if you still are having bleeding, take a sonogram. By then you should see a heartbeat. If not, there is a problem. If you start to have heavy bleeding with cramps, call your doctor right away. Good Luck and congratulations.
