Understanding Semen Analysis Results: Morphology Explained

Question
Hello again Dr. Ramirez,
I have a question regarding my husbands semen analysis, my doctor is away until mid August and I'm really anxious for an explanation of the results I was given by his receptionist. Here is what I was told over the phone:

Volume: 10.0
Motility: 85
Count: 21
Normal head shape- Less than normal range 60%

The first three parts I understand is normal, the one I questioned was the morphology. What does the last part mean? I'm not sure if less than 60% is bad or not. Thank you in advance!

Answer
Hello Laura from Canada,

Basically the semen analysis is completely normal.  The three things that we look at are count, motility and morphology (anatomy).  Count should be at least 20 Million/ml, motility should be at least 60% motile and morphology should be at least 30% normal.  With those parameters you can see that all the findings in your husband's analysis are normal.  I am not sure what your lab means by less that 60% for the morphology.  That is not the accepted way to measure morphology.  It is okay for it to be less than 60% as long as there are at least 30% normals.

The volume is quite extraordinary, though not clinically significant.  Most people only have 1-3 ml of volume.  10 ml is a huge amount.  That is more a comment and not a clinical statement.

Good Luck,
