AnswerTwin pregnancies/double baby care is exponentially harder than a singleton. And you're right organization is key. So work on that first. Take it one step at a time. Don't try to make lots of changes at once, you'll just get overwhelmed.
Sleep is critically important to health. Consistent lack of sleep will deplete your physical and psychological reserves quickly, then even the simplest of task will be very difficult to achieve.
So if you are not getting at least 6 hours of sleep a night, make that a priority.
Try to get some help with the babies when you are not working, that way you can get a grip on household management and actually have scheduled time to achieve your goals. Twin baby care is a 24/7 and you're working outside of the home too. It sounds like you barely have any free seconds in you weekly routine.
You say that "EVERYONE AT HOME FEELS IT IS A PART OF POST PREGNANCY," no, it isn't. Physically, our muscles don't pop back to "normal." But I question whether exercise should be a top priority for you now. Lots of moms of twins don't really come up for air until both babies are sleeping through the night, eating solid foods, and beginning to play on their own. Even here, you only begin to get little slices of time.
Knee pain indicates that your ligaments are still long and not supporting the joints as much as they should. Try to avoid movements that stress the joints. If the pain persists or gets worse, see your doctor. Heavy shoulders, well gee, it sounds like you're holding up your entire world, so no wonder.
My suggestions are:
get more help
work on organization, get a workable routine going, and stick to it
get more sleep
eat better, and eat the same amount of calories everyday, at the amount that supports your ideal body weight
when you babies are older and starting to get easier to manage, then start adding fitness activities into your weekly routine
