PCOS & Fertility: Tracking Ovulation with Medication & HCG - A Personal Experience

Question
Hi.. I am suffering from PCOS tring to conceive from 5 months. This is my third cycle of the medications to conceive where my doctor gave me ubiphene 100 from 2nd day of my periods for 5 days. I also underwent follicular study, on day 17th the biggest follicle on right ovary was 17mm n small ones on left. So my doctor gave me HCG injection on my 17th day evening. Unfortunately my husband fell ill that day and we couldn't be together on that day n the next day we tried only once and not again as he had bcome very weak coz of the fever. Today is my 20th day n I went to the follicular study n that 17mm follicle has grown to 24mm and the doctor said it has not ruptured. Its already 4 days since I took the HCG injection n the egg has still not ruptured.. is this possible? Are there few possibilities that the egg will not rupture at all?? Do I still have the chances of conceiving if we try being together today? We are very much wanting for a baby.. please help me out..

Answer
Hello Divyashree from India,

17 mms is adequate for triggering ovulation but a little earlier than most fertility doctors would use.  Most will trigger at 19-22 mms.  Also, looking at the follicle several days after the HCG does not rule out or rule in ovulation.  The ruptured follicle will often re-fill with fluid within 2-3 days after ovulation, forming the corpus luteum cyst.  There is the possibility that the ovary did not ovulate since the trigger was early, in which case it might ovulate at 24 mms.  I would recommend that you keep having intercourse for the next two to three days.

I would also recommend that your doctor consider using a higher dosage of the medication.  PCO patients tend to need higher doses to stimulate ovulation, closer to the 250 mg per day range.

Good Luck,
