Breast Pathology Report Analysis: Follow-up Ultrasound Findings

QUESTION: In December 2005 mammogram showed a "cluster" A core biopsy came back: right breast atypia.  January 2007 Surgical pathology report came back:Benign breast tissue with proliferative fibrocystic changes and microcalcifications, see microscopic description: Most sections from the submitted specimen show benign breast tissue with nonspecific fibrosis.  In the areas next to the prior biopsy site there are areas of usual ductal hyperplasia, apocrine metaplasia, and columnar cell changes without atypia.  A few midly dilated ducts are seen.  A few scattered microcalcifications are also seen. Occasional ducts exhibit florid usual dutal hyperplasia, with some areas showing a cicropapillary configuration.  However, the cells on the proliferative epithelium within the ducts are cytologically bland, with no evidence of clonality.  No areas of central necrosis are seen.  No areas of significant atypia are seen.
October 2007 mammogram report was clear but the ultrasound on right breast showed, as the doctor explained two "sticky spots" in a different area then before.  I will have a core biospy.  What does this sound like and does this have anything to do with the atypia cells from the cluster?  


ANSWER: Your previous biopsies show NO evidence of cancer though there are signs of something that eventually over time may have developed into one if it had not been removed. What the new biopsy will show we of course do not know yet! We will just have to wait for the result!

QUESTION: I was giving the previous biopsies for history only.  My surgeon explained them.  My question is: Does one have to do with the other?  I do not understand what a "Sticky spot" means from the ultrasound. Would that be the same as a cluster like before?

Possibly. You may well run a genetic risk of such developments over time. I'm a Swede living in Sweden and English is not my first language. I honestly do not know what the radiologist meant by that expression, sorry! When I translate it into Swedish no bell rings. The clusters were seen in mammograms and this is an ultrasound scan. 2 completely different modalities so really not comparable. And since I do not know the meaning of that expression I can not comment on it any more unfortunately.
