Irregular Periods & PCOS: Understanding Brown Discharge - Expert Advice

Question
Hi
I'm 22. When I was 16 I was diagnosed with PCOS, polycistic ovaries. I have NOT had a period since I was 16 years old, except one time.

The last time I had my period was the very first day of college 2 years ago, when I was so nervous and terrified I literally started my period that morning, but by the end of the day it was done.

However, every few months or so, I will have this strange goopy thick brown discharge.

What is this? I have it today and yesterday, it's been 2 or 3 months since I've had it.

It looks like "left over" blood.

Answer
The reason women with PCOS may have no periods is because they don't ovulate.  Ovulation causes the release of progesterone for about 10 days, and when the progesterone drops, the period comes on.  Women with PCOS don't tend to have progesterone, but they do have estrogen.  Estrogen is what causes the uterine lining to build up.  If the uterine lining builds up over time, eventually some bleeding will occur.  I think that's what the goopy brown discharge you're having is, some mild bleeding and perhaps secretions from an over built up uterine lining.  It's actually not good for you to have this continuous build up of your uterine lining, which can also lead to uterine cancer over time.  It can be prevented by taking progesterone pills for about 1 week every 2 months.  This will cause the uterine lining to clean off every 2 months, and will prevent cancer from developing.  I would talk to your doctor about this.  Best wishes.
