Why would a neurologist intake form ask if you have ever had an abortion? Would it possibly be linked to neurological issues later on? Why would doctors need this information and is it relevant to your quality of care?

The doctor does not need to know the information.  Abortion is not related to any neurological disorder later on.  Women with Multiple Sclerosis v=can have an exacerbation of disease symptoms during pregnancy.  They resolve after delivery or abortion.
