Endometriosis Chocolate Cysts: Symptoms, Causes & Treatment

Dear Doctor
I am writing to you from India. I am 34 years old and I have a son who is 5 years old. I have been married to 8 years. After my first child, I had 2 more pregnancies and I got them aborted myself. Now last year in January I conceived again, but miscarried in April automatically, this time I wanted to keep the baby.
I had to go through the DNC for the last one. When I got all the check ups done, it was diagnosed that I have endometriosis cyst(Chocolate) in my right ovary. Since April I have been trying but no results.
I am very very keen to have a child now, pls. help.

Hello Shazia from India,

Since you have been able to get pregnant several times in the past, it shows that everything probably can work.  More than likely the endometriosis is the problem.  I would recommend that you proceed with laparoscopy to remove the endometrioma (chocolate cyst) and surgically treat any other endometriosis.  The tubes can be checked at that time to make sure that they are still open.  I would then follow that with three months of Lupron depot 3.75 mg injections + Micronor tabs daily (add back).  After three months, you need to start trying for pregnancy urgently, otherwise if too much time passes, the endometriosis will return.  

If you have an endometrioma, we consider that stage IV endometriosis, and you may have difficulty from this point getting pregnant.  Many of these patients will have to turn to IVF in order to achieve pregnancy.

Make sure that you see a very good doctor for the laparoscopy so that the ovary is NOT removed.
