Methadone for Fibromyalgia Pain: Dosage Concerns & Drug Interactions

Question
would 40 mg of methadone 4 times a day be too high of a starting dose for pain - given to a 53 yr old person for fibromyalgia, that also has emphysema, copd, pnuemonia (changing from lortab to methadone) also taking xanax, soma, ambien, advair, bumex, geodon, ritalin, nexium and zithromax for the pnuemonia? Your opinion and advice would be greatly appreciated.
thanks-
Terry

Answer
It may be the correct dose if the person has a lot of real pain, but the drug combination does not appear to be rational since there appears to be several reactions of concern that would need to be changed. Drug doses for pain meds depend a lot on the severity of the pain medication and the prior pain med dose. OK?
