Epidural Injections for Lumbar Disc Herniation: Are They Worth It?

Question
Hi again Charles,

I have a herniated lumbar disc, L4-5. I had surgery (laminectomy) in 2003 after trying 2 epidurals that didn't work. The disc reherniated after 3 years of no pain. Before considering surgery again, we decided to try epidurals again. I had one last Monday and, so far, the sciatica hasn't improved. In your opinion, is it a waste of time to get another epidural? My thinking is if they didn't work 3 years ago, why would they work now. I hate to inject drugs into my body if they aren't effective. I'd rather just proceed with the surgery, which I know will work. The first time, the surgeon only shaved 18% of the disc material. I still have lots left. Thanks.

Answer
Hi Dan,
As you know the routine here is three injections per treatment. The epidurals are a lot less invasive than surgery . Surgery has attendant risks and therefore it shouldn't be rushed into. The second course of epidurals may work this time so I'd finish them before proceeding to surgery.
