Breast Imaging Results: Understanding Mammogram & Ultrasound Findings

QUESTION: I had Diagnostic Mammogram and Ultrasound,  Right Breast dont April 29 and I just got the result today. Please help me with the result.
RIGHT BREAST ULTRASOUND: There is 0.6 cm slightly irregular hypoechoic oval mass at 12 o clock, 1 cm above the nipple. This is likely a cyst with debris, fibroadenoma, or island of fibrocystic tissue, but could be malignant.
ASSESSMENT: Suspicious ABnormality (BIRADS 4)

DIGITAL SPECIAL VIEW MAMMOGRAM: RIGHT BREAST: Supplemental views do not confirm a suspicious mass in the central right breast. There are scattered densities consistent with small cyst, fibroadenoma, or island of fibroglandular tissue.

   I AM 43 YEARS OLD. NO FAMILY HISTORY OF BREAST CANCER. PLEASE HELP ME EVALUATE THIS RESULT. I WILL APPRECIATE IT VERY MUCH THANK YOU.

ANSWER: What is seen is probably NOT a cancer but based only on these pictures it is not possible to exclude the possibility of a cancer. Since your lesion is well seen by ultrasound I suggest an ultrasound guided needle biopsy of your lesion so that we will KNOW what it is! Good luck!

QUESTION: I saw the breast surgeon last week and I am sheduled for an Ultrasound guided biopsy next week. But I forget to asked the surgeon why is that that on my the Screening Mammogram, it showed 0.9 cm mass and then when I was called back for a Special view , the mass did not showed at all . BUT THEN I AM GOING FOR BIOPSY BECAUSE IT SHOWS A 0.6 CM MASS ON THE ULTRASOUND. THANK YOU AGAIN FOR YOUR HELP REGARDING THIS.


Answer
First of all mammography uses x-rays. If breast tissues are dense due to much fibrocystic tissues (as is probable here) mammography is less reliable. It works best for breasts with much fatty tissues. That is why an ultrasound scan - which works in another way, like radar but with sound waves - was used to give more reliable pictures. MRI breast scans are also very useful in this situation. My personal opinion is that mammography ALWAYS should be supplemented with ultrasound & MRI breast scans (BOTH)! Good luck!
